The Social Worker position requires understanding of common ethical and legal issues in social work practice in health care settings. This includes biopsychosocial‑spiritual assessment; use of the strengths perspective; client and family engagement in all aspects of social work intervention; case management/care coordination/health care navigation; discharge and transition planning; client concordance with and adherence to the plan of care; advance care planning;
palliative care, including pain and symptom management; hospice and end‑of‑life care; identification of child abuse, trauma, neglect, and exploitation; crisis intervention; facilitation of benefits and resource acquisition; advocacy within the interdisciplinary team and health care institution; client, family, interdisciplinary, and community education; family systems issues; life cycles; and caregiving roles and support needs. Shriners Hawaii is looking for a full‑time Social Worker to join our amazing team!
This position is full‑time, exempt, and benefits eligible. The pay range for this position is $70,054–$105,081 annually. Compensation is determined based on years of experience and departmental equity.
- Document assessments, interventions, plans, and evaluations in accordance with regulatory agency requirements and SHC policy.
- Coordinate the plan of care to achieve desired outcomes and integrate the work of the health care team by coordinating resources and services necessary to accomplish agreed‑upon goals.
- Advocate by supporting or recommending services for the protection of the patient's health, safety, and rights.
- Collaborate by working with patients, families, caregivers, and the health care team locally and within the system to jointly communicate, problem‑solve, and share accountability for optimal outcomes.
- Arrange services among community agencies, physicians, patients, families, caregivers, and others involved in the plan of care.

- Bachelor in Social Work
- SW license in state of practice
- Master's in Social Work
- 3 years Social Work
- Case Management Certification (CMC, ACM or ANCC)
- EMR (Epic) experience
