We are looking for an exceptional Speech Language Pathologist (CCC-SLP) to support the needs of short-term rehab and long-term care residents. Your goal is to integrate with the facility’s team, so communication skills are vital. You will have a flexible schedule, ample support from your Therapy Program Manager, competitive pay, and a rewarding work environment. You will connect deeply with patients/residents, and their lives and well-being will be improved by your work.

• Provide speech and language pathology services to residents in short-term rehab and long-term care settings.
• Assess, diagnose, and treat communication, swallowing, and related disorders in collaboration with the care team.
• Develop and implement individualized treatment plans aligned with residents’ goals.
• Communicate effectively with residents, families, and facility staff; provide education and coaching.
• Document progress, adjust care plans, and participate in care conferences as needed.

• CCC-SLP (Certificate of Clinical Competence in Speech-Language Pathology).
• Ability to work in a flexible schedule with PRN availability.
• Strong collaboration and communication skills; patient-focused and team-oriented.
