Home Town Pharmacy is seeking a dedicated Long-Term Care Staff Pharmacist to support our mission of serving patients, communities, and partner facilities with excellence. This role is central to our LTC operations and requires strong clinical judgment, communication skills, and a commitment to our culture of service.
What You’ll Do- Ensure accurate, clinically appropriate medication order processing
- Serve as the pharmacy’s drug information resource
- Perform clinical calculations, MTM activities, and Medication Regimen Reviews
- Oversee compounding, IV prescriptions for proper stability/dosing interval/flow rate/dispensed supplies, and regulatory compliance
- Support formulary and therapeutic interchange initiatives
- Participate in on‑call rotation
- (If applicable) Provide consulting services, chart reviews, facility reporting, and regulatory support
- Bachelor’s degree or higher in Pharmacy
- Michigan Immunization Certification
- CPR certification
- Strong communication, leadership, and relationship‑building skills
