Position Details

Full‑time and part‑time/per‑diem Hospice RN. The role requires on‑call responsibilities. The location is the Troy office, but you will serve the Kent County area and surrounding regions.

Responsibilities
  • Provide one‑on‑one care in the comfort of home.
  • Support patients and families with dignity and heart.
  • Spend real time where it matters most.
  • Care without the chaos of hospital halls.
  • Provide emotional support to help patients live their final days with dignity and comfort.
  • Conduct appropriate supervisory visits and clinical observation of team members.
  • Participate in interdisciplinary group meetings and provide feedback for patient/family plans of care.
Requirements
  • Current Michigan license in good standing.
  • Minimum 1 year hospice experience.
  • Empathetic approach to patients and caregivers.
  • Ability to adapt to patient status and needs on a full‑time on‑call basis.
  • Initial and ongoing assessment of patient's physical, functional, psychosocial, and environmental needs.
  • Implement the individualized plan of care and revisions as needed.
  • Compliance with Advisa Care Hospice policies and procedures, Medicare compliance.
  • Consulting with and educating patient/family on disease process, self‑care techniques, end‑of‑life care, and ethical concerns.
  • Supervision of paraprofessionals providing services to patients.
  • Preparation of clinical and progress notes, participation in IDG conferences.
  • Participation in hospice performance improvement program.
  • Maintain high level of customer satisfaction with clients, families, and caregivers.
  • Communication of information using current process and technology within the organization.
  • Participate in field visits daily, weekly office meetings as indicated.
