Whatyou will be doing
The MDS Nurse Case Manager is responsible for coordinating MDS assessments, overseeing discharge planning, and ensuring full compliance with regulatory guidelines. In this role, the Case Manager works closely with residents and families from admission through discharge, leading the MDS/RAI assessment process, completing required documentation on time, and maintaining accurate records. The Case Manager also ensures that each resident’s care is thoroughly assessed and supported throughout their stay.
The MDS Nurse Case Manager updates individualized care plans, conducts Care Conferences, and collaborates with residents, families, and the interdisciplinary team to develop effective discharge plans. This includes reviewing admission information, verifying alignment with physician orders, and facilitating communication with insurance providers and skilled nursing facilities to ensure smooth transitions. The ideal Case Manager brings strong communication skills, problem‑solving abilities, and a genuine passion for supporting others while delivering high‑quality, resident‑centered care.

- Nursing Degree (Associates or Bachelor’s) required.
- Active RN or LPN licensure in the State of Michigan.
- RAC‑CT certification, preferred.
- Proficiency with the MDS process is required.
