Per Diem and Part time Hospice Registered Nurse

We’re Hiring an RN – Hospice at Home with Advisa Care

• Provide one‑on‑one care in the comfort of home.
• Support patients and families with dignity and heart.

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement individualized plan of care and revisions as needed.
• Compliance with Advisa Care Hospice Policies and procedures, Medicare compliance.
• Consult and educate patient/family on disease process, self‑care techniques, end‑of‑life care, dealing with ethical concerns.
• Supervision of paraprofessionals providing services to patient.
• Prepare clinical and progress notes, participate in IDG conferences.
• Participate in hospice performance improvement program.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communicate information using current process and technology within organization.

Current on‑call hours: 5 pm – 8 am. Position requires on‑call responsibilities. Rotating schedule.

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily, weekly office meetings as indicated.
• Ability to adapt to patient’s status and needs on a full‑time on‑call basis.
