The West Michigan Hispanic Chamber of Commerce is looking for a passionate Nurse Care Manager to join our team in Kalamazoo. This role involves coordinating patient care across behavioral and physical health to ensure at-risk patients receive appropriate services and continuity of care.
This position offers excellent work/life balance with primary hours Monday through Friday, no holidays or weekends! The ideal candidate will have an Associate's degree in Nursing, an RN license in Michigan, and at least two years of relevant experience.
