The Avalon of Commerce Township is recruiting for the Assistant Director of Health Services, an LPN/LVN who is responsible for assisting with the overall supervision of nursing care in the community.

• Assist in supervising direct nursing care in the community.
• Supervise nursing personnel on all shifts, including staffing, scheduling, counseling, orientation, on‑call and evaluation of nursing staff.
• Administer direct nursing care when more than routine care is indicated.
• Conduct routine rounds to assure proper care and a safe, clean resident environment.
• Assure accuracy and completeness of medical records, including care plans and assessments.
• Coordinate, conduct, and document resident care plans and conferences in conjunction with or in the absence of the Director of Health Services.
• Respond to emergency calls as necessary.
• Act as Acting Director of Health Services in the director’s absence.
• Participate in family/resident counseling.
• Coordinate with the Move‑in Coordinator to ensure all regulatory documents are present prior to admission.
• Assure timely physician visits.
• Accompany physicians when making rounds.
• Notify physicians and families of changes in a resident’s condition.

• LPN currently licensed and in good standing in the state.
• Current CPR certification.
• Senior Care experience with a good understanding of regulations.
• Staffing/scheduling experience is preferred.
• Supervisory experience is preferred.
