RN – Hospice at Home

We’re Hiring an RN for Hospice at Home with Advisa Care. This is not rushed care but presence, compassion, and honoring each moment.

• Provide one‑on‑one care in the comfort of home.
• Support patients and families with dignity and heart.
• Spend real time where it matters most, caring without the chaos of hospital halls.
• Conduct supervisory visits to observe clinical performance of team members.
• Participate and provide feedback in interdisciplinary group meetings, and contribute to development, review, and revision of patient/family plan of care.
• Provide emotional support to help patients live their final days with dignity and comfort.
• Prepare clinical and progress notes, and participate in IDG conferences.
• Participate in hospice performance‑improvement program.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communicate information using current process and technology within the organization.

• Current Michigan RN license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to adapt to patient's status and needs on a full‑time on‑call basis.
• Initial and ongoing assessment of patient's physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with Advisa Care Hospice policies and procedures and Medicare compliance.
• Consulting with and educating patient/family on disease process, self‑care techniques, end‑of‑life care, and issues of ethical concern.
• Supervision of paraprofessionals providing services to patients.
• Participate in field visits daily, weekly office meetings as indicated.
• Current on‑call hours 5 pm–8 am; position requires on‑call responsibilities on rotating schedule.
