Position: NURSE CARE MANAGER (RN)

Responsibilities

As a Nurse Care Manager you will coordinate patient care across the behavioral and physical health spectrum. You will manage a caseload of at‑risk patients, acting as a liaison between ISK and other physical health providers to ensure appropriate services and continuity of care. Your duties include triaging and coordinating referrals and follow‑up, connecting patients to resources and programs, and assisting with metric compliance for CCBHC.

You will develop integrated health care plans and collaborate with other providers to meet care plan goals, while tracking outcomes for your caseload. The role offers a great work/life balance with primary hours Monday‑Friday and no holidays or weekends.

Requirements
  • Associate’s degree in Nursing (Bachelor’s preferred) and a valid RN license in the State of Michigan.
  • Valid driver’s license and reliable transportation.
  • Minimum two years of experience in primary care or mental health, preferably in a community setting.
  • Strong understanding of recovery‑oriented services for individuals with psychiatric needs, intellectual/developmental disabilities, or substance use disorders.
  • Intermediate computer skills (MS Word, Excel, Outlook).
  • Strong experience with electronic health records and ability to work within more than one system.

Workplace

Must be able to perform essential job functions with or without reasonable accommodations. Physical efforts include prolonged sitting and standing, light lifting up to 25 pounds, stooping, kneeling, crouching, or bending. The role requires coordination of hands and/or eye/hand/foot, ability to hear and communicate verbally, and use of a computer for periods in excess of 30 minutes. Working conditions are in an office environment with noise from computers, copy machine, and telephones.

Exposure to blood borne pathogens, infectious diseases, and parasites may occur. You may encounter community environments such as homes, schools, courts, and other settings that may or may not be barrier‑free. Travel throughout the Kalamazoo area may be required.
