• Collect comprehensive data pertinent to the patient’s health or situation
  • Assess the patient’s physical and developmental needs
  • Analyze assessment data to identify appropriate patient needs
  • Document nursing history and age‑appropriate physical/emotional needs, strengths for assigned patients
  • Review vital signs to assess condition and detect deviations from normal
• Analyze the assessment data to determine patient problems or issues

  • Confirm problems with patient and family when appropriate
  • Document expected outcomes and plan of care related to the problems through the clinical pathway and outcome teaching record
