Role Overview
As a Hospice RN Case Manager, you’ll provide skilled nursing care and compassionate support to patients and families during end-of-life care.
Key Responsibilities- Build rapport with patients, families, and interdisciplinary team members
- Coordinate comprehensive care addressing physical, emotional, and spiritual needs
- Work collaboratively to ensure comfort, dignity, and quality of life
Serving Kalamazoo, MI, and surrounding communities, with frequent travel to patients’ homes, assisted living, or nursing facilities.
Qualifications- Current RN license, at least one year of clinical nursing experience, reliable transportation, and valid driver’s license.
- Preferred qualifications include a BSN and hospice or home health experience.
- Strong nursing skills, ability to assess patient needs, manage multiple priorities, and respond effectively in home or facility settings.
$80,000–$85,000
