Role Description

This is a full-time or part-time, in home Nurse role based in Benton Harbor, Kalamazoo, Schoolcraft and Albion (Michigan). The Nurse will provide direct patient care in the home and community settings, including assessments, medication administration, wound care, and monitoring of vital signs and overall health status. Daily responsibilities include developing and implementing care plans, coordinating with physicians, therapists, and other healthcare professionals, and educating clients and families on disease management and safety.

The Nurse will document all care accurately, follow established clinical protocols, and uphold regulatory and company standards. The role also involves advocating for client needs, supporting transitions between care settings, and contributing to a positive, collaborative team environment.

Qualifications
  • Current, unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Michigan and compliance with all state and federal nursing regulations.
  • Demonstrated clinical skills in patient assessment, care planning, medication administration, wound care, and monitoring of chronic and acute conditions.
  • Strong communication and interpersonal abilities to educate clients and families, collaborate with interdisciplinary teams, and build trust with diverse populations.
  • Organizational and time-management skills to manage caseloads, prioritize care needs, and complete timely and accurate documentation in electronic health records.
  • Experience in home health, hospice, or community-based care is preferred; familiarity with Medicare and other payer regulations is beneficial.
  • CPR certification and valid driver’s license with reliable transportation to visit clients.
