Charge Nurse

Role:

The Charge Registered Nurse (RN) supports daily clinical care coordination for elderly participants in a community-based program offering comprehensive services through an interdisciplinary team.

• Overseeing clinical care workflows and providing leadership to the care team.
• Monitoring, assessing, and managing participants' health conditions in collaboration with providers.
• Administering medications, treatments, immunizations, and wound care.
• Conducting nursing assessments and contributing to care plans.
• Supporting staff onboarding, training, and quality initiatives.
• Managing inventory, equipment, and ensuring compliance with regulations.
• Participating in on-call and holiday rotations as needed.

• Current RN license and graduate of an accredited nursing program.
• Preferred: two years of leadership or Charge Nurse experience.
• At least one year working with elderly or long-term care populations.
• CPR certification or ability to obtain.
• Strong clinical judgment, communication, organizational, and leadership skills.
• Compassionate, participant-focused approach.

• Willingness to work beyond normal hours and in varied environments.
• Must meet CMS competency standards prior to independent work.
• Fully vaccinated or willing to be vaccinated against specified illnesses.
