Job Title
Social Worker at Maple Crest Health Center
Job SummaryAs our Social Worker, you will proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team. You will support residents and families through mental health and substance use disorder programs as well as home‑health service coordination.
Responsibilities- Assess, develop, and implement comprehensive plans of care with patients, families, and interdisciplinary team members.
- Collaborate with the Registered Nurse Manager for timely discharge planning and patient needs assessment.
- Evaluate psychosocial impacts of illness and provide direct care and counseling services.
- Maintain accurate documentation of patient interactions, care plans, and progress notes.
- Coordinate with community resources to support patient and family needs.
