Summary

The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care - positive customer service and the maintenance of an environment committed to serving the veteran.

Qualifications
  • United States Citizenship
  • English Language Proficiency; candidates must be proficient in spoken and written English.
  • Education:

    graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing (NLNAC) at the time of completion.
  • Verification: obtained from the State approving agency or the National League for Nursing.
  • Licensure: full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, Commonwealth, or District of Columbia.
  • History of license: if any license has ever been revoked, suspended, denied, restricted, limited, or placed in probationary status, appointment requires specific approval as per applicable regulations.
  • GS-3: none beyond basic requirements.
  • GS-4:
    Six months of qualifying experience as an LPN or LVN, or graduation from an approved school plus one year of experience in a supervised medical facility; or graduation from an approved school of at least 24 months duration.
  • GS-5:
    Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.
  • GS-6:
    Completion of at least 1 year of qualifying experience at the GS-5 level or equivalent.
  • Demonstrated Knowledge, Skills, and Abilities (KSAs) at each grade level (GS-5 and GS-6): full range of practical nursing care, collaboration with RN/MD, patient education, medication administration, emergency response, documentation, and advanced patient care skills.
  • Preferred Experience:

    1-2 years of Licensed Practical Nurse experience in an outpatient clinic setting.
  • Strong verbal and written communication skills with patients, families, and interdisciplinary team.
  • Skilled in managing multiple electronic health programs.
Duties
  • Completes focused data collection using all clinical reminders as outlined in the clinical guidelines and standing order policy.
  • Performs pre‑visit phone calls to prepare patients for upcoming appointments.
  • Maintains clinic workflow by screening patients in a timely manner and assisting other teams when needed.
  • Coordinates care after appointments and utilizes health coaching during pre‑visit calls or clinic visits.
  • Collects data from unscheduled patients and telephone calls when needed and contributes to the secure messaging process.
  • Provides health education on self‑management, prevention, and wellness to patients and families using motivational interviewing and health coaching techniques.
  • Administers medications, including immunizations and injections, and performs procedures such as EKG, Accu Check, phlebotomy, bladder scanning, and ear lavage.
  • Collaborates with coworkers and internal customers, demonstrating initiative and professional interaction.
  • Attends and participates in team huddles and weekly meetings, contributing to comprehensive patient care plans.
  • Ensures medication and supply expiration dates are checked regularly, maintains patient privacy, and keeps work area compliant with Joint Commission standards.
  • Follows specific guidelines covering all aspects of work assignments, policies, SOPs, and plans of care.
  • Participates in quality improvement initiatives and maintains ongoing education.
  • Identifies and addresses potential hazards or issues in patient care.
  • Advocates for patients to ensure highest level of care and support.
  • Manages time effectively, balancing multiple tasks while providing patient‑centered care.
  • Proficient in electronic health record (EHR) systems and familiar with telehealth practices.
  • Other duties as assigned.

Work Schedule: Monday through Friday, 8:00 AM – 4:30 PM. Telework:
Ad‑hoc Virtual. This is not a virtual position.
