This position provides practical nursing care to patients within the scope of practice and as applicable to assigned area(s).
Qualifications- Current Montana Licensed Practical Nurse (LPN) license required.
- Current BLS certification required as applicable to assigned area(s).
- Previous experience in a similar position preferred.
- Must have capability to connect to internet for daily data exchange.
- To be eligible for the Driver portion of the position:
Current Driver’s License required. - Driving record for the previous thirty‑six (36) months must not reflect any major driving offenses (such as DUI, convicted of a felony involving a motor vehicle, suspended license, three (3) or more speeding tickets or other moving violations, etc.).
- Personal vehicles used for LH business:
State law minimum liability coverage limits required. - Excellent organizational skills, detail‑oriented, a self‑starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
- Commitment to working in a team environment and maintaining confidentiality as needed.
- Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
- Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
- Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.
Specific Duties
- Provide skilled nursing care to patients utilizing the nursing process as appropriate for LPNs in the assigned area(s).
- Exhibit a commitment to professionalism, civility, teamwork, and customer service while undertaking all tasks and interactions with others.
- Exhibit knowledge of policies and procedures as they relate to the specific job duties, patient care and safety, and as applicable to assigned area(s).
- Exhibit knowledge of staffing standards and a demonstrated commitment to cost containment with all resources.
- Operate and maintain equipment to departmental standards as applicable to assigned area(s).
- Maintain nursing standards of care and execute appropriate provider orders.
- While executing patient care, exhibit knowledge of and maintain the patient’s right to diversity, privacy, and confidentiality.
- Perform a standardized focused assessment or reassessment of patient at regular intervals appropriate for the condition of the patient under the direction of the RN and as ordered by the provider.
- Administer medications, treatment, and provider‑specific care to patients in compliance with nursing policies/procedures, LPN scope of practice, and as applicable to assigned area(s).
- Communicate all appropriate and critical changes in the patient’s condition, status, and findings to the appropriate stakeholders in a timely manner as defined by policies and procedures and/or provider orders as applicable to assigned area(s).
- Document all clinical findings and required regulatory mandates in the medical chart in compliance with policies and procedures or as defined by regulatory requirements as applicable to assigned area(s).
- Contribute to maintaining an updated, individualized plan of care that includes goals, interventions, medications and visit frequency orders as applicable to assigned area(s).
- Maintain a thorough knowledge of infection control procedures and strictly adhere to policies and procedures.
- Maintain regular and consistent attendance as scheduled by department leadership.
