Live Well Home Care is seeking a Per Diem/PRN Registered Nurse (RN) to coordinate Start of Care, manage re-certifications, discharges, and resumption of care in the Kalispell area. You will provide direct patient care, administer medications, and monitor vital signs while collaborating with physicians to implement care plans.
You will educate patients and families about medical conditions and preventive care, assist with discharge planning, and support post-discharge care.
