AWARE PATH housing navigators earn $47,000.00 per year.
Responsibilities- Help individuals who are homeless, at risk of homelessness, or have serious mental illnesses/co-occurring disorders apply for Social Security disability benefits.
- Identify and support client in securing permanent housing and assist with placement in transitional housing as appropriate.
- Inspect apartments using HUD's standard checklist to ensure safety standards are met.
- Assist with communications with landlords and provide document assistance with lease and other housing documents; provide housing retention aide to client of the program.
- Conduct appointments with clients pertaining to housing needs, community options, social service referrals and document client-related interactions.
- Provide housing case management to clients, aligning them with appropriate service providers to develop and implement outreach activities including networking with other community-based organizations, support groups, religious organizations, etc.
- Participate in outreach activities for high-risk groups; respect client rights and personal preferences in treatment are essential.
- Document records in HMIS system.
- Complete SOAR training.
- Additional duties as assigned.
- A BS/BA Degree in a Human Services Field and minimum of one (1) year of direct work experience or an education equivalency through a combination of education and pertinent employment experience.
- Previous experience with homeless outreach preferred.
- Must have, or be willing to obtain, a valid Montana Driver’s License with an acceptable driving record, as well as an acceptable criminal and protective services background check.
