About the Position
Job Opportunity – Home Health Case Management RN – Hawaii

Compensation: $88/hour – Details provided to qualified applicants
Schedule:
Rotating weekends as needed (2-3 per month); 6 on-call shifts per month including weekends
Highlights:
Home health case management role serving the Leeward side and central regions
Collaborate with interdisciplinary teams to optimize patient outcomes and coordinate care transitions
Focus on complex patients including frequent ED utilizers, chronic pain, and substance abuse cases
Utilize Homecare Home Base and OASIS documentation systems
Mileage reimbursement provided at $0.43 per mile
Travel assignment with professional growth opportunities in home health nursing
Requirements:
Active Hawaii RN license (must be obtained prior to start date; allow 15-20 business days for processing)
Minimum 2 years Medicare-certified home health agency experience required
Previous travel nursing experience required
Proficiency with Homecare Home Base and OASIS required
Active and clear driver's license with current auto insurance (personal vehicle required)
Current COVID and Flu vaccinations required (declinations not accepted)
Bachelor's degree in Nursing preferred
Case management experience preferred
