Center Well Home Health, part of Humana Inc., is seeking a detail-oriented Scheduling Coordinator to support our in-home care teams. In this role, you will coordinate and optimize daily visit schedules for nurses, therapists, and other clinicians, ensuring patients receive timely, high-quality care 'll manage appointment confirmations, handle changes, and maintain accurate information in our EMR and scheduling systems. As a key communication link between patients, families, and field staff, you'll provide exceptional customer service, resolve scheduling issues, and support compliance with care plans.

Responsibilities

  • Coordinate and maintain daily home health visit schedules for clinicians and patients
  • Confirm, update, and optimize appointments based on patient needs and clinician availability
  • Serve as primary scheduling contact for patients, families, and care teams
  • Enter and update scheduling data accurately in EMR and scheduling systems
  • Communicate schedule changes promptly to field staff and patients
  • Collaborate with clinical leaders to ensure coverage and timely patient visits
  • Monitor workload, visit frequency, and priorities to meet care plans and compliance standards
  • Assist with reporting, documentation, and administrative support for scheduling operations
