As a full‑time therapist at Center Well Home Health, you will play a vital role in helping patients regain strength, mobility and independence—all from the comfort of their homes. By delivering personalized care focused on rehabilitation and functional improvement, you’ll empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.

Responsibilities
  • Assess and screen a patient’s daily living/work‑related skills; develop therapeutic retraining programs with measurable objectives, and administer and interpret diagnostic and prognostic tests of function to assist the physician in evaluating the patient’s level of function.
  • Confer with the patient’s physician and other health‑care team members, and participate in the development and revision of the Plan of Care and treatment plan.
  • Provide therapeutic treatment and instruction in accordance with physician orders to improve or restore strength, coordination, range of motion, and function—or to teach compensatory measures.
  • Review and expand retraining programs in a manner consistent with each patient’s behavioral goals and the overall rehabilitation program guidelines.
  • Consult with other vocational team members to develop and implement vocally oriented plans that match patients’ needs and capabilities.
  • Report patient status and progress to the physician, rehabilitation staff, clinical manager, and family members.
  • Participate in care coordination, discharge planning, and serve as a resource to other health‑care personnel.
  • Design community reintegration activities to support physical reconditioning and psychological adjustment, coordinating the plan with interdisciplinary team members.
  • Recommend and/or design special adaptive equipment to improve a client’s residence, work environment, or participation in the rehabilitation program and community.
  • Design and train staff and family members to carry out the retraining program, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings.
  • Coordinate, oversee, supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
  • Accurately and thoroughly document patients’ care observations, interventions, and evaluations on the day services are rendered; ensure interim verbal orders are properly documented and implemented; submit evaluation, treatment plans, and discharge summaries to the supervisor and care‑management staff.
Required Experience & Skills
  • Degree from an accredited Occupational Therapy program.
  • Minimum of six months of occupational therapy experience (preferred).
  • Home Health experience is a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • Valid driver’s license, auto insurance, and reliable transportation.
Compensation & Hours

Pay Range: $54.00 – $76.00 per visit/unit; $85,400 – $117,500 per year (base pay).

40 hours.
