Position: CARDIOLOGY AMBULATORY CARE PHARMACY SPECIALIST

The Cardiology Ambulatory Care Pharmacy Specialist is a highly trained clinical pharmacist responsible for providing comprehensive, evidence-based medication management for patients with cardiovascular disease in the ambulatory care setting. This specialist will function as an integral member of the multidisciplinary cardiology team, collaborating with cardiologists, advanced practice providers, nursing staff, and other healthcare professionals to optimize patient outcomes.

The pharmacist will provide direct patient care through collaborative practice agreements and/or established protocols, focusing on chronic cardiovascular disease management, medication optimization, patient education, and quality improvement initiatives. Areas of practice may include, but are not limited to, heart failure, hypertension, hyperlipidemia, anticoagulation, cardiometabolic disease, and guideline-directed medical therapy (GDMT) optimization. Demonstrates flexibility with assignments within professional scope/duties/licensure. As a salaried professional, you may be expected to work above your FTE.

  • Provides comprehensive medication management services for cardiology patients in an ambulatory clinic environment.
  • Evaluates and optimizes cardiovascular medication therapy in accordance with current evidence-based guidelines and organizational protocols.
  • Initiates, adjusts, and monitors medication therapy under collaborative practice agreements as applicable.
  • Supports management of disease states including:
  • Heart Failure with reduced or preserved ejection fraction (HFrEF/HFpEF)
  • Guideline-directed medical therapy (GDMT) initiation and optimization
  • Hyperlipidemia and ASCVD risk reduction
  • Cardiometabolic therapies, including GLP-1/GIP and SGLT2 inhibitor optimization when clinically appropriate
  • Orders and assesses appropriate laboratory monitoring according to scope of practice and established protocols.
  • Provides patient-centered medication education focused on adherence, safety, affordability, and therapeutic goals.
  • Identifies and resolves medication-related problems including adverse effects, drug interactions, and barriers to therapy.
Collaboration & Clinical Integration
  • Serves as the pharmacy expert within the cardiology care team.
  • Partners with cardiologists and advanced practice providers to improve access, medication optimization, and achievement of clinical quality measures.
  • Participates in interdisciplinary care planning and transitions of care initiatives.
  • Supports population health efforts through proactive identification of patients requiring medication optimization.
Program Development & Quality Improvement
  • Assists with development, expansion, and evaluation of cardiology pharmacy services.
  • Develops clinical protocols, workflows, and evidence-based treatment pathways.
  • Tracks and reports clinical outcomes and program performance metrics, including:
  • GDMT optimization rates
  • Blood pressure control
  • Lipid goal achievement
  • Medication adherence
  • Patient access and clinical interventions
  • Leads and participates in quality improvement projects that enhance cardiovascular outcomes.
  • Provides medication-related education to patients, providers, pharmacists, residents, students, and healthcare team members.
  • Serves as a preceptor for pharmacy residents and students as applicable.
  • Maintains expertise in evolving cardiovascular literature, guidelines, and best practices.
  • Participates in committees and initiatives related to cardiovascular care, medication safety, and pharmacy practice advancement.
Required Education/Licensure
  • Doctor of Pharmacy (PharmD) degree from an ACPE-accredited College of Pharmacy required.
  • Current pharmacist license or eligibility for licensure in the state of practice required.
Must Meet One Of The Following Requirements
  • Completion of a PGY2 Ambulatory Care Pharmacy Residency with demonstrated cardiology experience

OR

  • Board Certification in:
  • Board Certified Ambulatory Care Pharmacist (BCACP)

OR

  • Board Certified Cardiology Pharmacist (BCCP)
Required Experience
  • Previous experience managing cardiovascular disease states in an ambulatory care or specialty clinic environment preferred.
  • Experience practicing under collaborative drug therapy management agreements preferred.
  • Experience with heart failure medication optimization and cardiology clinic workflows strongly preferred.
Preferred Education/Licensure
  • PGY2 Ambulatory Care Residency with focused training in cardiology clinics.
  • Board Certified Cardiology Pharmacist (BCCP).
