Overview

Riverside Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our clinic team in Kankakee, Illinois. In this role, you will provide high-quality nursing care to patients in a clinic setting, supporting both routine and acute care needs. You will collaborate with physicians, other healthcare professionals, and patients to deliver comprehensive care, ensure optimal health outcomes, and enhance the overall patient experience.

• Patient Care:
  Assess, plan, implement, and evaluate patient care based on individual needs, clinical judgments, and provider orders. Administer medications, treatments, and vaccinations as prescribed.

  Educate patients and their families about health conditions, treatment plans, and preventive measures. Provide clear instructions on medication usage, self-care practices, and follow-up care.
• Clinical Procedures:
  Perform and assist with various clinical procedures and tests as applicable, including blood draws, wound care, and EKGs.
• Documentation:
  Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and progress notes in electronic health records (EHR) systems.
• Coordination of Care:
  Collaborate with providers, specialists, and other healthcare providers to coordinate patient care and follow-up. Facilitate referrals and communication between patients and other healthcare services as needed.
• Patient Advocacy:
  Serve as an advocate for patients, addressing their concerns and ensuring that their needs are met with empathy and professionalism.
• Infection Control:
  Adhere to infection control protocols and standards to ensure a safe and hygienic environment for patients and staff.
• Inventory Management:
  Maintain clinical supplies and equipment by ensuring stock levels, expediting orders, and verifying receipt of supplies.
• Regulatory Compliance:
  Ensure clinic compliance with DNV regulations and maintain accurate documentation of all required clinical patient information during the rooming process.
• Adhere to AIDET format and standards. Acknowledge and greet all customers timely with a professional and friendly demeanor.

• Administrative Support:
  May include assisting with scheduling appointments, managing patient flow, and performing other administrative tasks as needed.
• Training and Development:
  Participate in ongoing professional development and training programs.
• Community Outreach:
  Engage in community health initiatives and outreach programs as assigned.
• Patient Support:
  Provide emotional, psychological, and spiritual support to patients, families, and friends.
• Medication Management:
  Maintain and administer medications accurately, including sample logging in EMR and medication room management.

• Minimum of 1–2 years of nursing experience, preferably in a clinic or outpatient setting.
• Experience with electronic health records (EHR) systems is desirable.
• Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities, with a patient-centered approach.
• Compassionate, detail-oriented, and able to work effectively both independently and as part of a team. Ability to manage multiple tasks in a fast-paced environment while maintaining high standards of care.

• Graduate of an accredited Licensed Practical Nurse (LPN) program.
• Current and unrestricted LPN license in the state of Illinois.
• Basic Life Support (BLS) certification.
