Overview

Riverside Medical Center is seeking a dedicated and experienced Registered Nurse (RN) to join our Skilled Nursing Facility (SNF) team. The RN provides high‑quality nursing care, manages patient care plans, and coordinates with interdisciplinary team members to ensure optimal outcomes. The position is on‑campus and requires flexibility within the professional scope and licensure.

• Assess, plan, implement, and evaluate individualized care for residents.
• Administer medications and treatments as prescribed, documenting all care accurately.
• Monitor resident health conditions, including vital signs, symptoms, and treatment responses.
• Provide wound care, IV therapy, and other specialized nursing interventions as needed.
• Collaborate with physicians, nurse practitioners, occupational therapists, physical therapists, and social workers to develop and implement comprehensive care plans.
• Communicate effectively with residents and families regarding care plans, treatment options, and progress.
• Maintain accurate and detailed records of resident care, ensuring compliance with facility policies and regulatory standards.
• Participate in audits and reviews, contributing to quality improvement initiatives to enhance patient care and safety.
• Educate and support residents and families regarding health management and preventive care.
• Mentor less experienced nursing staff and assist with training new employees.
• Respond promptly to medical emergencies, implementing emergency protocols and documenting incidents according to facility policies.
• Assist with scheduling and managing nursing staff to ensure adequate coverage and effective care delivery.
• Participate in staff meetings, care conferences, and other facility-related activities.
• Adhere to federal, state, and local regulations, including infection control, safety, abuse, neglect, and resident privacy (HIPAA).

• Minimum of 2–3 years of nursing experience, preferably in a skilled nursing or long‑term care setting.
• Strong clinical skills and effective communication abilities.
• Proficiency in patient assessment and care planning.
• Ability to work independently and as part of a team, committed to high‑quality, resident‑centered care.

• Graduate of an approved school of nursing.
• Current Registered Nurse (RN) license in the state of Illinois.
• Basic Life Support (BLS) certification.

• Professional nursing certification in a specialty area.
• Basic ECG recognition skills.

The RN must be able to:

• Stand, walk, bend, and lift up to 60 lbs frequently.
• Perform repetitive hand use, such as fine manipulation and arm reaching, for extended periods.
• Respond to medical emergencies that may involve rapid movement and manual handling.

USD $34.18 – $44.45 per hour.
