Overview
The Flow Coordination Nurse optimizes patient throughput by coordinating admissions, transfers, and discharges in collaboration with nursing units, providers, bed management, and ancillary departments. The role monitors hospital capacity and patient flow in real time, proactively identifies barriers to movement, and supports safe, efficient transitions of care.

Nursing leadership, Bed Management, Case Management, Physicians, Therapy Services, and Support Services
Patient Flow Coordination
- Monitor capacity and patient flow in real time and address barriers.
- Facilitate admissions, transfers, and discharges across units.
- Collaborate with bed management and nursing leadership to ensure appropriate placement.
- Prioritize admissions based on acuity, capacity, and organizational needs.
- Implement real‑time solutions to maintain flow.
Admission & Discharge Oversight
- Coordinate direct admissions and transfers from internal and external sources.
- Coordinate discharge readiness and assist during high‑volume periods.
- Escalate barriers to timely discharge (e.g., clinical readiness, transportation, placement, diagnostics).
Interdisciplinary Rounds (IDR) & Care Progression
- Conduct IDR on weekends to support care progression & discharge planning.
- Review and facilitate key care elements: oxygen weaning, diet advancement, electrolyte management, elimination status, ambulation & mobility goals.
- Escalate discharge/transfer barriers during rounds.
Interdisciplinary Collaboration
- Work with physicians, case management, therapy services, and ancillary departments for effective communication and discharge planning.
- Act as liaison among nursing, providers, environmental services, transport and ancillary departments.
- Participate in daily throughput or bed huddles and provide real‑time updates.
- Proactively communicate to align placement, readiness, and resource availability.
Operational Efficiency
- Support key metrics: LOS, discharge before noon, ED boarding time, and bed turnaround time.
- Use data dashboards to identify trends and improvement opportunities.
- Recommend and support implementation of process improvements.
Clinical Support
- Provide direct patient care support during peak admission/discharge periods.
- Ensure compliance with regulatory and organizational standards during transitions.
- Reinforce patient and family education regarding discharge instructions.
- Experience in charge nursing, throughput, bed management, or case management roles.
- 2–3 years of acute‑care nursing experience.
- Strong clinical judgment and prioritization.
- Advanced communication and interdisciplinary collaboration.
- Ability to manage multiple competing priorities in a fast‑paced environment.
- Systems thinking focused on patient flow and operational efficiency.
- Problem‑solving and critical‑thinking skills.
- Illinois RN licensure.
- BLS certification.
- Bachelor’s degree in Nursing (BSN) from an accredited program.
