Overview
Riverside Healthcare is seeking a motivated and compassionate License Pending Licensed Practical Nurse (LPN) with a license pending to join our Skilled Nursing Facility (SNF) team in Kankakee, Illinois. The LPN provides nursing care to residents under the supervision of a Registered Nurse (RN) while awaiting full licensure. This role delivers direct patient care, assists with daily living activities, and ensures high-quality care in compliance with facility policies and state regulations.
The LPN performs tasks within their scope of practice as defined by the facility and under the supervision of licensed nursing staff.
- Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, and eating under supervision.
- Monitor residents’ vital signs (temperature, blood pressure, pulse, respiration).
- Report changes in residents’ conditions to the supervising nurse immediately.
- Provide basic wound care, administer prescribed medications under supervision, and assist with treatments as directed by licensed nursing staff.
- Direct Patient Care: perform nursing tasks within scope of practice under RN supervision.
- Resident Assessment and Monitoring: observe changes in physical, emotional, or mental conditions and report findings to the RN or healthcare team; assist with data collection for assessments and care planning; document health status and care provided in medical records.
- Medication Administration: assist with medication administration under direct supervision; monitor for adverse reactions and report concerns; organize medication carts and supplies per facility standards.
- Collaboration and Communication: work with the interdisciplinary team; participate in care plan meetings and shift reports; communicate with residents, families, and healthcare professionals to address needs.
- Documentation and Compliance: maintain accurate records of care provided; ensure compliance with regulations; participate in audits and quality improvement.
- Resident Advocacy: advocate for residents’ rights and well-being; support informed decisions and address complaints promptly and compassionately.
- Professional Development and Licensure: study for the LPN licensure examination; attend required trainings and in-service programs; seek mentorship while awaiting licensure.
- Patient and Family
Education:
provide basic education about health conditions and care under supervision; assist with educational materials. - Organizational Compliance: adhere to regulations (infection control, safety, privacy); mentor staff and participate in training; complete LMS modules as required.
- Graduate of an approved school of nursing.
- Proof of passing the NCLEX-PN.
- Submission/payment to IDPFR for Illinois LPN license.
- Current BLS certification.
