Position: CARE MANAGER RN

Overview

Riverside Healthcare is seeking a dedicated and compassionate Care Manager Registered Nurse to join our dynamic team. This role is integral in coordinating patient care, improving health outcomes, and ensuring a seamless transition through various levels of care. The ideal candidate will possess a blend of clinical expertise, strong organizational skills, and the ability to advocate for patients and their families.

Additionally, this position requires flexibility with assignments within professional scope/duties/licensure.

Essential Duties
  • Patient Care Coordination:
    Develop, implement, and manage individualized care plans for patients with complex health needs. Collaborate with multidisciplinary teams to ensure comprehensive and effective care. Completes management functions including patient/family education. Refers reviewed cases to a Physician Advisor as needed.
  • Assessment and Planning:
    Conduct thorough assessments of patients medical, psychological, and social needs. Create and adjust care plans based on ongoing evaluations and patient progress.
  • Patient Advocacy:
    Serve as a primary contact for patients and their families. Provide education on health management and resources, ensuring patients understand their care plans and treatment options.
  • Case Management:
    Monitor and evaluate patient care and outcomes, coordinating with healthcare providers to address any issues or changes in patient condition. Ensure timely follow-up and continuity of care.
  • Documentation:
    Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes. Ensure compliance with regulatory requirements and organizational policies.
  • Resource Management:
    Identify and facilitate access to community resources and support services for patients. Assist in managing care costs and optimizing the use of healthcare resources.
  • Quality Improvement:
    Participate in quality improvement initiatives to enhance patient care and optimize processes within the care management program.
Non-essential Duties
  • Administrative Support:
    Assist with administrative duties related to care management, such as preparing reports and attending meetings.
  • Training and Development:
    Contribute to the training and orientation of new staff members or interns as needed.
Responsibilities Required Experience
  • Minimum of 3-5 years of experience in a clinical setting.
Preferred Experience
  • Experience in managing complex cases and working with diverse patient populations is highly desirable.
  • Preferred previous experience with case management and utilization management.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, organizational, and interpersonal skills.
  • Proficiency with electronic medical records (EMR) and care management software.
  • Ability to work independently and as part of a team. Strong commitment to patient-centered care and continuous professional development.
Required Licensure/Education
  • Current Registered Nurse (RN) license in the state of Illinois.
  • New Graduate RNs starting on or after 1/1/25 must earn Bachelors of Science in Nursing (BSN) within 3 years of start date.
Preferred Education
  • Bachelors Degree in Nursing (BSN) or higher from an accredited institution.
  • Certification in Care Management (CCM) or similar certification is preferred.
