Moments Hospice is seeking an RN Case Manager to coordinate care for patients at home and on the road. You will conduct initial assessments, refine care plans, provide hands-on nursing care and educate patients and families, while collaborating with the healthcare team.
You must hold an RN license in your state, be proficient with EMR software, and possess strong planning and communication skills. A valid driver's license and reliable vehicle with insurance are required.
