General Responsibilities- 1. Provide intake, screening, assessment, case planning, crisis intervention, and case management services.
- 2. Determine eligibility for Illinois Department on Aging Waiver Program (Community Care Program).
- 3. Conduct comprehensive needs assessments and determine eligibility for services.
- 4. Establish written care or treatment plans with clients and families for maximum independence.
- 5. Active intervention and advocacy to access necessary services from various programs.
- 6. Complete monitoring, follow-up, and proper termination of cases.
- 7. Ensure the completion of Intensive Casework with each completed comprehensive assessment.
- 8. Complete critical incident reports in accordance with Illinois Department on Aging Standards.
- 9. Maintain confidential case records and meet documentation standards.
- 10. Accurate completion of all required reporting and documentation.
- 11. Maintain cooperative relationships with service providers and community organizations.
- 12. Conduct educational presentations for professional and community groups.
- 13. Attend training, staff, and team meetings, and act as an Agency ambassador in the community.
- 14. Other duties as assigned within the guidelines of this position.
