Home Health Licensed Practical Nurse – PRN – Center Well (Humana)

Contract type:
Pay per visit/unit and full-time salary for 40 hours per week.

Schedule:

40 hours/week (full time).

Pay per visit/unit: $33.00 - $47.00

Base pay range (full-time): $53,000 - $73,000 per year.

• Provide skilled nursing care in patients' homes under RN direction.
• Implement individualized care plans collaboratively.
• Educate patients and families on disease management, treatments, and self-care.
• Maintain accurate records of patient care, documenting visits, interventions, outcomes.
• Maintain contact with patients, physicians, clinical managers, and team, addressing patient/family needs and status changes.
• Participate in care coordination activities and discharge planning.

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance, reliable transportation.
• Current CPR certification.
• Two years experience as an LPN/LVN in a clinical setting.

• Nursing experience in a Home Health or Hospice setting.

TB Statement:
This role is patient-facing and subject to Humana's Tuberculosis screening program. TB screening required.

Driving Statement:
This role is part of Humana's driver safety program and requires a valid state driver's license and personal vehicle liability insurance. Vehicle insurance must meet minimum state limits or higher.
