POSITION SUMMARY
This permanent part-time position (32 hours/week) is responsible for facilitating the Community Health Improvement Plan’s progress through community partners, and the Healthy Harvey Coalition and its corresponding Leadership Teams and subgroups, managing the Health Department’s compliance to funders through reports and tracking, and assisting with internal policy revisions.
ESSENTIAL FUNCTIONS

• Collaborate with community organizations and agencies, safety organizations, and other health organizations in Harvey County and across Kansas to further the mission of increasing the health, safety, and success of all people in Harvey County.
• Maintain and build working knowledge of community engagement, relationships, and partnerships.
• Work closely with the Information/Community Outreach Specialist to evaluate areas of facilitation need in the County.
• Participate in the County’s Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP) processes. Align coalition work to identified health priorities of CHNA and action steps of CHIP with a focus on policy, systems and environmental change for longevity.
• Assist in sustainability planning, which may include grant writing, for the continued impact of the CHIP initiatives.
• Gain support in funding and other resources for the continuation, expansion, and sustainability of a healthier Harvey County.
• Facilitate, with the Healthy Harvey Coalition, the development and/or implementation of proven community-based strategies to address the CHIP and Social Determinants of Health.
• Encourage recruitment efforts and orientation of new community members of the coalition, Leadership Teams, and subgroups.
• Judiciously monitor grants for deliverables and fulfill reporting duties.
• Account for grant-hired parties’ responsibilities.
• Monitor the budget of grant funding with the assistance of the Fiscal Management Coordinator.
• Track the outcomes and impact of partners and coalition activities to evaluate the benefits and return on investment for the people of Harvey County.
• Create a schedule to review department’s internal policies working closely with assistant director and director.
• Review, suggest changes, and revise policies after supervisor approval.
• Maintain records (minutes, agendas, etc.) for coalition.
• Coordinate meetings and distribute minutes of the Healthy Harvey Coalition.
• Manage social media and website, including posts of meeting announcements, special events, and minutes as required.
• Ensure Open Records and Open Meeting Acts regulations are followed for appropriate department functions.
• Provide surge capacity to Public Health Emergency and Epidemiology for investigative work and community containment in a public health emergency or severe outbreak.
• Other duties as assigned.
MINIMUM REQUIRED QUALIFICATIONS
• Bachelor’s degree from an accredited college or university in Sociology, Public Health, Community Development or related field with three (3) years’ work in public health, community development or related experience/training.
• Fluency in Spanish preferred.
• Must possess a valid driver’s license.
SUPERVISORY RELATIONSHIPS
Works independently with general guidance from the Director to ensure coordination of objectives and priorities of the department. Works under the guidelines of various Federal and State governmental regulations as well as under the County’s policies, procedures, municipal code, ordinances and various other regulations.
