ResponsibilitiesEssential Duties and Responsibilities (includes, but is not limited to):
- Coordinate and manage individualized patient care plans from admission through discharge
- Perform ongoing patient assessments and monitor progress toward goals of care
- Facilitate discharge planning, including post-acute placement, home health, DME, and community resources
- Collaborate with physicians, nursing staff, social workers, and ancillary departments to ensure continuity of care
- Educate patients and families on treatment plans, discharge instructions, and available resources
- Participate in interdisciplinary rounds to proactivelyaddress barriers to discharge
Before we go any further, we do have some deal-breakers. You must have:
Required qualifications:
- BSW or other bachelor's degree in a clinical healthcare-related field.
- At least one year of experience in case management, discharge planning, or nursing management
- LVN/LPN graduate from an accredited school of nursing, with a current LVN/LPN license, and 2 years of acute care experience.
- Experience and knowledge in basic to intermediate computer skills.
Preferred qualifications:
- Minimum 5 years of acute care experience preferred.
- Case management certification within 2 years of hiring.
- At least one year of experience in case management, discharge planning, or management is preferred.
- Current BLScertificate preferred.
- Knowledge of Milliman Criteria and Inter Qual Criteria preferred.
