This full-time Ambulatory Clinic RN Care Coordinator role focuses on providing patient‑centered care for individuals with pelvic floor disorders, urinary incontinence, prolapse, and postoperative recovery.

• Serves as a clinical liaison among patients, providers, staff, and community resources to ensure high‑quality care.
• Supports patient education, care planning, triage, and care navigation, balancing hands‑on nursing and back‑office duties, including nurse‑led procedures and provider assistance.
• Manages patient flow, coordinates care, and promotes self‑management, especially for high‑risk populations with multiple comorbidities.
