Position Summary

The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert providing ethical, evidence-based, safe and compassionate nursing care. The role focuses on patient safety and quality of care by identifying and clarifying patient needs, performing clinic‑related patient care, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes. The RN Care Coordinator provides support to assigned physicians by communicating with patients, families, and caregivers and relaying information to the physician, verifies all orders placed during the clinic session have been completed, and notifies the physician of results.

Responsibilities

and Essential Job Functions
  • Responsible for management of patient flow, triage, and direct patient care.
  • Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
  • Manage the care of the patient population by developing expected patient outcomes, defining appropriate resource utilization, and working with the provider to set patient goals.
  • Provide nursing support/coordination to the practice to optimize patient outcomes and improve patient quality of life through clinical and educational programs and/or support services.
  • Support patient self-management of disease.
  • Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
  • Manage high‑risk patient care, including management of patients with multiple comorbidities or high risk for readmission to a hospital setting, including a registry.
  • Develop relationships with patients and competently use assessment skills to identify physical, psychosocial, financial, and environmental health care needs of the patient.
  • Educate the patient and appropriate ancillary personnel about the health care needs, safety issues and benefits which include patient education.
  • Utilize available electronic tools, such as EMR, to efficiently document assessments, clinical findings, plans of treatment, and progress toward goals.
  • Observe, record and report patient’s condition and reaction to drugs and treatments to physicians; educate patient/family about diagnostic procedures, medications, nutrition and maintenance of health and wellness.
  • Maintain/review patient records, charts, and other pertinent information; record tests and examination results and follow up with patient; post tests and examination results.
  • Respond to/refer incoming patient treatment‑related phone calls; instruct patient and family regarding medications and treatment instructions.
  • Maintain patient confidentiality.
  • In providing patient care, document using EMR to maintain and/or develop patient records in compliance with practice standards.
  • Perform the professional, clinical and/or technical competencies of the assigned unit or department.
  • Other duties may be assigned as required.
Required

Education and Experience
  • High School Graduate
  • Bachelor Degree Nursing
  • Two or more years of RN experience.
Preferred

Education and Experience
  • Previous medical office experience.
Required Licensure and Certification
  • Licensed Registered Nurse (LRN) – Multi‑State – State Board of Nursing; must have current active multistate RN license in good standing in the state in which you live. Staff hired prior to 2/1/2024 are required to obtain a multistate RN license from the state in which they live at the time of their next RN license renewal by 4/30/2026.
  • Basic Cardiac Life Support (BLS or BCLS) – American Heart Association (AHA) within 14 days.
Knowledge Requirements
  • Basic typing and word processing ability.
  • Excellent analytical, critical thinking and problem‑solving skills.
