The Nursing Facility Behavior Supports Program Manager leads KDADS’ implementation and management of the nursing facility pilot within KS’ Rural Health Transformation Program (RHTP) grant from the Centers for Medicare and Medicaid Services (CMS). The role is focused on supporting nursing facilities in addressing complex resident behaviors associated with dementia and other cognitive disorders. This position is responsible for establishing and overseeing program processes, including recruitment of clinicians and others with local subject matter expertise, as well as eligibility determination and care planning.
The Program Manager works directly with nursing facilities in identifying residents for whom additional supports to the nursing facility staff could be beneficial and assist them in securing those supports. This will involve their outreaching and then linking subject matter experts to work with the nursing facility staff. It will also include providing consultation and access to training for the participating nursing facilities.
The role supports facilities in strengthening care practices that address complex behaviors, while aligning with Centers for Medicare and Medicaid Services nursing facility requirements and antipsychotic reduction initiatives.
The position also has oversight of and works closely with the contractor who will be developing a Medicaid add-on that is based upon the program pilot.
Job Responsibilities may include but are not limited to the following:Program Management and Eligibility Determination
- Determine and approve eligibility and care plan, including application of established criteria and professional judgment in complex cases. Develop, refine, and maintain eligibility criteria, processes, and documentation standards. Work with nursing facilities to assess needs and identify appropriate supports for residents with complex behaviors associated with dementia.
- Manage day-to-day implementation and operation of the initiative, including coordination of activities, timelines, and deliverables.
- Collaborate with RHTP leadership and internal staff to support program alignment with overall initiative goals and requirements.
- Engage directly with nursing facilities participating in the Rural Health Transformation Program to assess needs, provide consultation, and support implementation of appropriate evidence-based and person-centered approaches to care.
- Assist nursing facilities in identifying training and consultation needs and facilitate access to resources aligned with RHTP goals. Deliver training and provide consultation to nursing facility staff as appropriate.
- Assist nursing facilities in connecting with subject matter experts to support development of care strategies, including those that reduce reliance on psychopharmacological interventions.
- Support nursing facilities in identifying and addressing complex behaviors of pilot participants through non-pharmacological approaches and alignment with best practices in dementia care.
- Recruit subject matter experts in all areas of the state who may be able to provide training, technical assistance, and clinical consultation to participating nursing facilities about how to address complex behaviors of pilot participants.
- Serve as a connector between subject matter experts and participating nursing facilities.
- Oversee and monitor contract with entity who will assist KDADS and KDHE in developing a Medicaid add-on based upon nursing facility pilot. Ensure that all KDADS contracting requirements are met.
- Work closely with contractor and keep them informed as to the progress of the pilot. Identify successful program elements that should be included in the Medicaid add-on.
