We are seeking a Registered Nurse (RN) Home Health Case Manager, Hospice to provide compassionate nursing care for clients facing terminal diagnoses.
Key Responsibilities Include- Collaborating with an interdisciplinary team to develop and implement patient-centered care plans.
- Supporting patients and families through education, guidance, and comfort measures.
- Conducting assessments, ongoing evaluations, and addressing physical, emotional, and comfort needs.
- Supporting healthcare decision-making and goals of care discussions.
- Active Kansas or Compact RN license.
- Hospice experience preferred; training provided for the right candidate.
- Valid driver's license and auto insurance.
- Strong organizational, interpersonal, and managerial skills.
- High energy, motivation, and the ability to perform effectively in demanding situations.
- $5,000 sign-on bonus and competitive pay.
