Salary Range: $54,000.00 - $57,000.00 Salary/year
Starting Salary: $54,000 - $57,000 (Salary)
Bonus: $2,000 ($1,000 Sign-on bonus will be paid on your first paycheck and the $1,000 Retention bonus will be paid after 12 months of service.)
Must be flexible with work hours, evening hours, and on-call.
We are seeking a High Acuity Case Management Specialist to join our team.
We are seeking a High Acuity Foster Care Case Management Specialist to join our team. Your role will have a high degree of autonomy in carrying out case management services as required by contract/licensing/funding body/accreditation requirements. You will exercise discretion and independent judgment in making decisions on the overall management and delivery of services to children on their caseload, aimed at achieving safety, permanency, and well-being.
You will serve as the legal guardian of the children on their caseload, making decisions and recommendations—occasionally on an emergency basis—to ensure the safety and welfare of those children. As a member of the Kansas Foster Care Case Management Services team, you will work with 5 other team members and report to our Manager of High Acuity Permanency Services.
- Engage in family finding to identify and take advantage of relative/kinship placement options for children in care
- Continually analyzes and assesses each family and child situation on an individual basis, using advanced knowledge in child welfare to develop recommendations regarding supportive services and resources each child and family needs
- Facilitate service planning with the child, family, and other treatment team members in accordance with Cornerstones of Care and DCF policy
- Coordinate any supportive services and resources children under case management services may need to ensure timely and efficient service delivery
- Bachelor’s degree in social work or other related field is required.
- At least 21 years of age and pass background check, physical and drug screening
- A valid driver’s license in the state you reside in, proof of current vehicle insurance and reliable transportation
