Schedule

The scheduled working hours for the Admissions Associate are set by the admissions Coordinator; this includes evenings, weekends, and some holidays.

Job Summary

The Admissions Associate supports new residents as they transition into the community and assists the admissions Coordinator with administrative duties, including admitting new residents and assisting with hospital referrals.

Preferred Experience
- Has familiarity with PCC and CRN.

Essential Functions / Duties
  • Facilitate a welcoming transition for residents into the community/facility
  • Greet new admissions upon arrival and orient them to the facility
  • Distribute welcome gifts and admissions information
  • Check in regularly with residents throughout their stay, liaise with other departments to ensure that residents feel welcome and assist in resolving resident concerns
  • Performs activities related to the transition of residents out of the facility: distribute discharge surveys, place follow‑up calls, send thank you cards, etc.
  • Serves as an assistant in increasing the occupancy of the community/facility by conducting tours and assessments for medical professionals, prospective residents, and their family members
  • Oversees aspects of event planning and promoting, attending community outreach efforts, and other events; includes the delivery of gifts
  • Prepares, screens, interviews, secures signatures, and/or receives information from prospective residents for admission to the health care facility; upon request, or as directed
  • Provides communication between residents, family members, staff, and other medical professionals regarding admission and/or the discharge process
  • Monitors admission of residents; ensures admission and/or discharge paperwork and related documents are completed and in the resident’s file
  • This includes the following tasks:
    • Ensure that the resident’s room is ready prior to occupancy date
    • Notify appropriate staff of the admission and communicate any pertinent information to relevant departments
    • Verify necessary transfer forms have been completed and obtained
    • Prepare all paperwork necessary for admission and enter information into the designated computer system and obtain resident picture, if applicable
    • Compute first month’s rent and community fee dues, if applicable
    • Meet with the resident and/or family to review facility policies and procedures, sign paperwork, and collect appropriate rent/fees
    • Orient resident and family with facility layout
    • Follow‑up with the resident and/or DPOA for any missing information
  • Establish and maintain good relationships with hospital discharge coordinators and social service consultants; establish open lines of communication with consultants and consistently take follow‑up action on recommendations
  • Follow‑up with resident and/or family after admission for any concerns and forward to appropriate staff
  • Record hospital referrals into the designated computer system for monthly reporting
  • Take inquiry calls regarding facility and services, follow‑up on pending inquiries
  • Provide coverage for admissions office when Admissions Coordinator is offsite
  • Assemble marketing materials such as packets and welcome baskets and maintain inventory of necessary supplies
  • Comply with federal, state, and company policies, procedures, and regulations
  • Operate computers and programmed with software needed to record, store, and analyze resident information; operate copy/fax/printer machines to send documents and produce documents
  • Perform other related duties as directed by the Administrator such as attending staff meetings, classes, and committee meetings as assigned or required
