• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in clinical quality improvement projects.
• Work with interdisciplinary teams to ensure holistic care.
• Contribute to projects and innovations in care delivery.
• Exercise independent clinical judgment in patient management.
• 2 years' or more experience in value-based care or managing high-acuity geriatric patients.

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of practice.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.

• Onsite outpatient clinical setting in Kansas City.
• Reporting to the Regional Medical Director.
• Standard schedule:
  Monday–Friday, 8:00 AM – 5:00 PM.

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.

Center Well Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment and more. Our unique care model focuses on personalized experiences, taking time to listen, learn and address the factors that impact patient well-being. Our integrated care teams, which include physicians, nurses, behavioral health specialists and more, spend up to 50 percent more time with patients, providing compassionate, personalized care that brings better health outcomes.
