Director, Patient Navigation

Summary

The Director of Patient Navigation is a leadership role responsible for the strategic oversight and operational management of patient navigation services across ambulatory and inpatient care settings. This leader will ensure a seamless, patient-centered experience through a concierge approach that prioritizes system integrity, prevents unnecessary patient leakage, optimizes pre-procedural care, and reduces hospital readmissions. The Director will collaborate closely with physicians and the care team to deliver high-quality outcomes and drive organizational excellence.

• 8-10 years clinical experience.
• 4-6 years hospital leadership in supervisor, coordinator, or management position.
• Minimum 5 years full time involvement in patient navigation, care coordination, or related healthcare leadership roles.

• Ability to facilitate effective meetings.
• Ability to analyze complex data, system, and problems, and determine possible solutions.
• Ability to display and analyze time series data using statistical tools.
• Excellent verbal and written communication skills, engaging in direct, open, honest interactions and feedback with a willingness to listen, teach and learn.
• Must be self-motivated and work independently without close supervision.
• Must possess the ability to work well with other professionals and staff in a creative and challenging work environment.

• Demonstrate technical competence with MS Office Suite.

RN (Registered Nurse) State of Missouri Preferred:
