Job Description

Our client is seeking a compassionate Home Health LPN to join their extraordinary care team. The ideal candidate will have an Associate degree, LPN license in the state of Missouri, and two years of LPN experience. Responsibilities include collaborating to provide patient care plans, supporting patients in the local area, and demonstrating proficiency with technology tools.

• Collaborate to provide management and delivery of patient care plans
• Support patients in Kansas City, MO
• Meet critical deadlines for charting
• Self‑manage time and schedule efficiently
• Demonstrate experience with tablets, mobile phones, and EMR software
• Schedule and Shift Details

Work schedule and shift expectations will be provided during onboarding.

Travel – occasional travel may be required within the local area.

• Associate degree or higher
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • Tru Chart EHR
• Valid Licensed Practical Nurse (LPN) in the state of Missouri (MO)
• ANY of the following valid licenses/certifications:
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
  • American Heart Association Basic Life Support (AHA BLS)
• Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all
