As a Licensed Practical Nurse (LPN) for PACE KC, you will be part of a team of caregivers to provide care for the people who need it most. Under the direction and supervision of the Medical Clinic Manager, you will provide general and skilled nursing care to participants, execute interventions in accordance with the plan of care, implement primary care orders, evaluate participant complaints in person and/or by telephone, develop an appropriate plan of care, provide participant, caregiver and family education, and communicate participant's condition to primary caregivers, physician and care team.
PrimaryFunctions
- Function as a member of the Care Team. Maintain regular attendance at, and participate in, Care Team meetings; communicate participant changes, collaborate on plan of care decisions and coordination for twenty‑four (24) hour care delivery. Effectively communicate with teammates during meetings or one‑on‑one conversations.
- In coordination with the Medical Care Manager, Registered Nurse and other nursing staff, develop and implement a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintain an aligned, comprehensive plan of care for all participants living in residential facilities and coordinate care between facility and Care/Interdisciplinary Team.
- Perform general and skilled nursing care to participants as assigned. Implement the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observe, record, and report participant's condition and reaction to drugs and treatments to providers. Triage participants to determine priority of care and assist in the treatment, examination, and testing of participants.
- Participate in nursing on‑call rotation to provide after‑hours and weekend assistance and care, as needed, to PACE KC participants.
- Receive participant complaints, triage, and respond to participant needs.
- Review and sign off orders and disseminate orders and instructions to appropriate staff and contracted personnel. Maintain and review participant records, charts, and other pertinent information. Complete documentation and charting in accordance with organizational policy.
- Working with the Medical Care Manager, perform care coordination with outside providers and facilities including hospitals, nursing homes, assisted living facilities, and physician/provider offices. May accompany participants to specialist appointments or make hospital or other facility visits when necessary for coordination of participant care.
- Provide treatment instructions and nursing education for participants and family.
- Maintain accountability for high customer service standards and monitor, evaluate, and recommend opportunities for improved participant and family/caregiver satisfaction.
- Manage and review participant's records, charts, pertinent information and staff documentation. Document as needed in medical records according to PACE KC standards.
- Maintain the stability and reputation of PACE KC by ensuring all activities and operations are performed in compliance with local, State, and Federal laws, regulations, and contractual requirements.
- Establish and maintain a productive working relationship with relevant contracted providers.
- Responsible for adherence to program cultural standards (Fundamentals) including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.
- Follow and ensure that the clinical team follows all organizational policies and procedures and CDC and OSHA safety guidelines.
- Educate staff regarding care and treatment for the elderly as needed.
- Act within the scope of authority to practice.
- Handle potentially infectious specimens with appropriate biohazard precautions, and practice Universal precautions.
- Protect privacy and maintain confidentiality of all company procedures, results and information about employees, participants and families.
