As a team member of the Senior Community Care (SCC) team, you will provide care to patients at home, nursing homes and assisted living for senior housing. The core of this role is the Patient Connect visit, a one‑time comprehensive visit for patients recently discharged from a skilled rehabilitation facility. The purpose of each visit is to facilitate a smooth transition to home and prevent readmission.
You will capture STAR measures, educate the patient, and document all acute and chronic diagnoses.
The NP will not be managing care or issuing orders, but will communicate pertinent findings to attending and facility staff. You will have the autonomy to work independently while collaborating with the care team.
Primary Responsibilities- Perform comprehensive assessments and document findings in a concise and compliant manner that meets CMS regulations.
- Ensure all diagnoses are coded accurately in ICD‑10 and documented appropriately to support the diagnosis at that visit.
- Address and document all quality elements as required by the APS.
- Conduct an initial medication review.
- Attend and complete all mandatory education and computer/virtual training requirements.
- Travel between care sites is mandatory.
- Care Coordination
- Communicate with the patient and necessary team members regarding any medical concerns during the visit.
- Program Enhancement Expected Behaviors
- Exhibit original thinking and creativity in developing new and improved methods and approaches to concerns/issues.
- Function independently and responsibly with minimal need for supervision.
- Enter available hours into the web‑based application at least one month prior to work time.
- Demonstrate initiative in achieving individual, team, and organizational goals and objectives.
- Participate in SCC quality initiatives.
- Check Optum email intermittently for required trainings, communications, and monthly scheduling.
- Active and unrestricted Nurse Practitioner license in the state of Missouri.
- Certified Nurse Practitioner through a national board:
Graduate of an accredited master’s degree in nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), Adult‑Gerontology Acute Care Nurse Practitioner (AG‑AC NP), Adult/Family or Gerontology Nurse Practitioner (AC‑NP), with preferred certification as ANP, FNP, or GNP. - Ability to move a 30‑pound bag in and out of a car and navigate stairs and varied dwelling conditions.
- Availability to work 10 hours per week.
- Access to reliable transportation to travel to client and/or patient sites within a designated area.
- Proficient computer skills, including the ability to document medical information in written and electronic medical records.
- 1+ years of nursing home experience.
- Experience meeting the needs of patients with complex behavioral, social and/or functional needs.
- Understanding of geriatrics and chronic illness.
- Understanding of advanced illness and end‑of‑life discussions.
The hourly pay for this role ranges from $90.00 to $155.00 per hour based on full‑time employment. In addition to salary, we offer a comprehensive benefits package, incentive and recognition programs, an equity stock purchase plan, and a 401(k) contribution.
