We are looking for a Registered Nurse (RN) for our team in Missouri.
— This is a hybrid role that requires fieldwork to visit patients’ homes in Kansas City, MO, and surrounding areas, with the possibility of occasional commuting to our Kansas City, MO office.

Position Overview:
Personal Care Services is a caregiving arrangement in which a patient has a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in the patient’s home that will provide them with a range of care options as their needs evolve.
The service is designed to provide options for alternative long‑term care to persons who meet nursing facility‑level care needs while preserving the dignity, self‑respect, and privacy of the patient.
In this position, as a licensed practical nurse, you will perform clinical evaluations in the home of the Personal Care Services member to determine the level of service to qualify for the program. You will also conduct monthly visits with members who are On Care with Freedom Care to ensure they are receiving quality service and address any changes in their condition.
EveryDay You Will:
- Provide support to the caregiver and member, and review services according to scope of the Personal Care Services program.
- Respect client and employee rights and privacy, ensure the security of protected information, practice in an ethical manner and be compliant with the agency’s compliance program and privacy policy.
- Follow Freedom Care’s clinical policy and procedures.
- Assess and regularly reassess the client’s needs and their PCP summary form, treatment and education and communicate any changes to the member’s PCP.
- Develop, implement, evaluate, revise, and provide input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provide instruction of care to Caregiver according to the plan of care.
- Complete all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.
- Actively coordinate the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources.
- Interpret the philosophy and objectives of the agency to clients, families, and others.
- Perform other job‑related duties as may be required or directed by the supervisor.
- Licensed Registered Nurse licensed by Missouri Board of Nursing.
- Minimum of 2 years clinical experience, 1 year of experience with elders or adults with disabilities.
- Bilingual in Spanish is a plus.
- Experience with CRM (Salesforce is a plus).
- Valid driver’s license with an insured vehicle for travel within the state.
- Understanding of regulatory and provider compliance requirements.
