Position Overview

We are seeking a compassionate, detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Care Coordinator
. Operating within a Federally Qualified Health Center (FQHC) environment, this role serves as a crucial bridge between patients, families, and healthcare providers to deliver holistic, patient-centered care.

Key Responsibilities Direct Patient Care & Care Coordination
  • Care Planning: Develop, implement, and monitor individualized care plans using the Patient-Centered Medical Home model to ensure quality and continuity of care.
  • Provider Support: Deliver dedicated individual nursing support for clinic providers, assisting with daily operations, managing chronic and acute conditions, and executing physician orders.
  • Clinical Nursing: Conduct comprehensive health, social, and behavioral assessments; direct routine clinical nursing care; administer vaccinations and prescribed medications.
  • Patient Advocacy & Triage: Serve as the primary point of contact for patients and families, facilitating communication, managing medication alerts, coordinating referrals, and managing triage.
  • Education: Educate patients and families on disease management, self-care strategies, and medication adherence in both individual and group settings.
  • Documentation: Maintain precise, timely, and compliant patient records in the Electronic Medical Records (EMR) system following HIPAA and data privacy guidelines.
Community Outreach & Specialized Programs
  • Health Promotion: Participate in health promotion and disease prevention initiatives tailored to the community’s unique needs.
  • Specialized Health Initiatives: Support and manage specialized health programs such as Vaccines for Children (VFC), Show Me Healthy Women, Ryan White, Primary Care Home Health, Infection Control, STD monitoring, and chronic disease programs (Diabetes, Hypertension, Hepatitis C, HIV).
  • Community Integration: Collaborate with local agencies and participate in health fairs, screenings, and educational events to enhance community healthcare resources.
Quality Assurance & Compliance
  • Maintain compliance with healthcare standards, OSHA, safety guidelines, and FQHC regulations.
  • Ensure proper vaccine and medication storage, handling, log management, and administration according to established clinical protocols.
  • Participate in quality improvement initiatives to enhance patient safety and clinic workflows.
Required Qualifications
  • Licensure: Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Missouri.
  • Experience: Open to all experience levels, including entry-level / fresh graduates willing to learn in a fast-paced environment.
  • Cultural

    Competency:

    Comfort and desire to serve a highly diverse patient population across all age groups (infants to geriatrics), including low-income households, underserved populations, and the LGBTQIA+ community.
  • Communication: Strong verbal and written communication skills in English. (Bilingual Spanish fluency is strongly prioritized for select openings).
Preferred Qualifications
  • Bilingual

    Skills:

    Fluency in Spanish (Highly Preferred).
  • EHR

    Experience:

    Familiarity with Electronic Health Record systems, particularly eClinical Works .
  • Software Proficiency: Basic working knowledge of Microsoft Office Suite.
