Responsibilities- Administer medications and monitor patient health as per care plans.
- Provide essential nursing care and support to clients in their homes.
- Assist with daily living activities, including bathing, dressing, and meal preparation.
- Communicate effectively with clients, families, and healthcare teams.
- Document patient care and progress accurately and in a timely manner.
- Educate clients and families on health management and wellness.
- Participate in care planning and contribute to team meetings.
- Maintain a safe and clean environment for clients at all times.
- Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Missouri.
- Previous experience in home health care or private duty nursing preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate demeanor with a focus on patient-centered care.
- Basic Life Support (BLS) certification required.
- Flexibility to work various shifts including weekends and PRN.
- Reliable transportation and valid driver's license.
