DESCRIPTION
POSITION SUMMARYThe Manager of Nurse Care Coordination (RN) is responsible for the leadership, development, implementation, and oversight of KC CARE's Nursing Programs. This position provides supervision of Nurse Care Coordinators, and assigned nursing staff while ensuring patients receive comprehensive, patient-centered care that addresses medical, behavioral health, social, and community-based needs. Serving as the operational leader for Population Health nursing services, the Manager provides clinical oversight, workflow development, quality monitoring, and staff development.
In partnership with the Vice President of Population Health, the Manager supports population health strategies, value-based care initiatives, quality improvement efforts, and compliance with organizational, grant, payer, and regulatory requirements. The Manager also serves as a clinical resource for complex patient needs, fosters interdisciplinary collaboration, and leads initiatives that improve patient access, care transitions, chronic disease management, sp ecial ty serv ices , and overall health outcomes.

Ensures compliance with and supports organizational performance for Patient-Centered Medical Home (PCMH), Primary Care Health Home (PCHH), HRSA, Medicaid, value-based care, grant-funded initiatives, and other applicable federal, state, and payer programs through effective care coordination, care management, quality improvement, and population health activities
Provide direct patient care as needed, including conducting assessments, developing treatment plans, and delivering nursing interventions.
Maintain accurate and timely documentation of patient care in the Electronic Health Record (EHR) system.
Foster a patient-centered approach, ensuring that all patients receive compassionate and respectful care.
Provides direct supervision and leadership of Population Health nursing staff and nursing programs.
Oversees nursing workflows related to care management, chronic disease management, specialty services, transitions of care, preventive services, and population health initiatives.
Ensures nursing programs operate in accordance with evidence-based practice, organizational policies, and regulatory requirements.
Partners directly with the Vice President of Population Health to establish departmental goals, strategic priorities, quality initiatives, and workforce planning.
