HCA Healthcare, a leading provider of healthcare services, is seeking a dedicated RN Case Manager to join our Acute Rehab team within Healthcare & Medical Services. In this role, you will coordinate patient care plans, facilitate interdisciplinary communication, and support safe, timely discharges. You'll collaborate closely with physicians, therapists, and families to optimize patient outcomes. Responsibilities

  • Perform comprehensive nursing and case management assessments for acute rehab patients, including clinical, functional, psychosocial, and discharge needs
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary rehabilitation team
  • Coordinate and lead discharge planning, arranging appropriate post-acute services, equipment, and follow-up care
  • Communicate and collaborate with physicians, therapists, nurses, social workers, and external providers to support safe, efficient care transitions
  • Educate patients and families on rehabilitation goals, treatment plans, medications, self-care, and community resources
  • Conduct utilization review activities and interact with payers to support authorization and appropriate length of stay
  • Identify and help resolve barriers to discharge, escalating complex clinical or social issues as needed
  • Maintain timely, accurate, and compliant documentation of assessments, care plans, progress, and discharge summaries
  • Participate in quality improvement, patient safety, and performance initiatives within the acute rehab unit
  • Adhere to all regulatory, accreditation, and organizational policies, promoting HCA Healthcare's culture of compassion, teamwork, and patient-centered care
