Job Description:The Score 1 RN (PRN) supports the goals of Score 1 for Health by coordinating and providing pediatric health screenings and screening events; maintaining patient flow; supervising and supporting screenings conducted by medical and allied health students; and ensuring adherence to regulatory requirements, universal precautions, and applicable policies and protocols. The RN provides timely follow-up with families, including health education, resource information, and assistance connecting to needed care and services;
cultivates relationships with community partners and stakeholders to promote and strengthen the program; and contributes to the ongoing development and operations of Score 1 for Health and the department.
Essential Duties and Responsibilities
Health Screening
- Coordinates and supports pediatric health screening events in schools and community settings
- Performs health screenings in accordance with established Score 1 for Health protocols, policies, and clinical standards
- Coordinates patient flow to promote safe, efficient, and timely completion of screening activities
- Supervises and supports medical, nursing, and allied health students participating in health screenings
- Provides instruction, guidance, and clinical oversight to students and volunteers performing screening activities
- Identifies abnormal or concerning screening findings and ensures appropriate follow-up according to established protocols
- Maintains accurate and timely documentation of screening results and related activities
- Assists with preparation, organization, and management of screening equipment and supplies
- Ensures compliance with infection prevention practices, universal precautions, privacy requirements, and applicable regulatory standards
Family & Community Outreach
- Conducts timely follow-up with families whose children have screening findings requiring additional evaluation, care, or resources
- Provides families with health education and information to support understanding of screening results and recommended next steps
- Assists families in identifying and connecting with appropriate healthcare providers, community services, and other resources
- Identifies potential barriers to care and works collaboratively with families and community partners to facilitate access to needed services
- Builds and maintains relationships with schools, healthcare providers, community organizations, and other partners supporting Score 1 families
- Participates in school and community outreach activities to increase awareness of Score 1 for Health and available health resources
- Collaborates with the Score 1 team to identify opportunities to improve screening processes, family follow-up, outreach, and access to care
Qualifications
- Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN)
- Current, unrestricted RN license in Missouri and/or Kansas, as required for assigned work locations
- Experience in pediatric, school-based, community health, or public health settings preferred
- Experience with patient/family education, health promotion, care coordination, or community resources preferred
- Bilingual in Spanish is a plus
- Proficiency with Microsoft Office, including Word and Excel
- Strong verbal and written communication and interpersonal skills, with the ability to effectively interact with children, families, students, school personnel, healthcare professionals, and community partners
- Strong organizational and time-management skills with the ability to prioritize responsibilities, manage multiple tasks, and work effectively with minimal supervision
- Detail-oriented, dependable, and self-motivated, with sound judgment and the ability to maintain confidentiality and exercise appropriate discretion
